Open the consultation form
Select Book a consultation in the site header or open the contact page. The same form works on desktop and mobile.
Tell the team how to reach you
Enter the required contact details and briefly describe the health goal or program you want to discuss. You do not need to choose a program before sending the form. Avoid including urgent or highly sensitive medical information in a general website enquiry.
Send and follow up
Review the details, submit the request, and wait for the clinic team to contact you. If your need is urgent, contact an appropriate medical service directly rather than waiting for a website response.
